Michigan Medicaid Card Replacement: Requesting a New Card

After your Michigan Medicaid application is approved, a mihealth card is mailed to you and any other Medicaid recipients in the household. This is a plastic insurance card with your name and Medicaid identification number displayed on the front. Although the card is intended to be permanent, you can request a replacement card if yours is lost, stolen or damaged. Replacement requests must be made over the phone.

Step 1

Call the Beneficiary Help Line at 800-642-3195.

Step 2

Follow the prompts to listen to the options. As of 2015, there's no specific option for replacing a mihealth card. You'll need to select option 4, which is "all other issues" then option 5, "your mihealth card."

Step 3

Inform the agent you want to request a replacement mihealth card. You'll need to verify your identity by providing your name, address, date of birth and Social Security number. Once that's confirmed and your request has been processed, the card will be sent to the address on your Medicaid account.
